Some Insurance Plans accepted!

We are contracted with some commercial insurance companies when plans are provided through an employer. Plans through the state or federal marketplace, or through Medicare or AHCCCS are not within our insurance contract. While there is crossover of company names and subsidiaries, our claims will not be paid by the insurance if the plan is not an employee-based plan. Clients will be responsible to pay out of pocket for all claims that are not paid by their insurance. Thank you!

Our Location

We are located about a half of a mile south of Banner Ironwood Hospital, at the light leading into the medical business park Pinal Professional Village. We are the second building on the left side of the entrance road. We have a west facing door, just next to Stauffer Dental.
